Graduate in Medicine from the University of Valladolid and Specialist in Anesthesia and resuscitation via MIR. In 1985 he passed the examinations for Inspector of Health Services of Social Security.  ![Mutua Fraternidad-Muprespa]()F.M.: It is the responsibility of the department to establish and maintain the necessary means for the provision of health care. What is the most laborious?

M.B.T.: On the one hand, we must carefully assess the resources we have and take care of the conditions in which they are found in order to be able to complement them when necessary. In any case, own resources require monitoring of their operation, both the equipment of the centers and monitoring of the corresponding authorizations for their facilities, including X-rays, equipment, medication storage, staff training, etc.

Everything that we cannot do with our resources we have to complement with external resources, and that is what we do with the establishment of health agreements.

F.M.: How are care centers arranged?

 M.B.T.: The process of establishing health arrangements begins by determining the needs we have at all times. Given that our own healthcare centers have resources and means (general practitioner, nursing, traumatology and physiotherapy, where they exist), the rest of the needs must be covered with external means, guaranteeing at all times that our injured people, wherever they are, can receive healthcare if they need it urgently. In this context we need support centers with a geographical distribution that guarantee emergency care to our workers, we need to cover other medical specialties (ophthalmology, cardiology, allergology, ENT...), we need a hospital where our traumatologists carry out surgical interventions, etc.

Once the needs are determined, you must choose the supplier, negotiate prices and sign a contract to which, in addition to the center's operating authorization, certificates of being up to date with payments in AEAT, TGSS are added. The process is completed with the authorization of DIGOSS, which is currently carried out electronically with the CAS@ transmission.

F.M.: Healthcare centers must have authorization for health operations, which varies depending on the locality. Is this equivalent to quality certifications?

 M.B.T.: No, it has nothing to do with it. The authorization to operate a center is an essential requirement to be able to operate; it is the document issued by the competent health authority in each Autonomous Community that indicates that the center meets the necessary requirements to do what the owner intends to do in the center. Quality certifications are voluntary processes to which the center undergoes in the line of continuous quality improvement.

F.M.: What other health coverage services does the department cover?

M.B.T.: As I have mentioned, ultimately it is about controlling that we have the necessary infrastructure, both our own and external resources, to be able to provide quality healthcare to our workers.

F.M.: Regarding collaboration with other Mutuals and after several years of experience, what have been the main advantages and disadvantages?

M.B.T.: Collaboration agreements with other mutual societies have undoubtedly been an invaluable tool in our environment. On the one hand, it guarantees an adequate healthcare provider, since it uses the same operating criteria, with a very favorable cost for both parties, which has led to significant savings in many cases, and better use of resources.

F.M.: The Mutual Member Ombudsman is another agent with whom we collaborate in the management of health complaints and claims. Could we talk about profiles or trends of those affected? And by locality?

M.B.T.: Indeed, all claims that are presented to the Mutual Ombudsman related to the health field go through this department, to inform them about it. And of course I have always thought that complaints are a very important source of information about what happens in our centers, thus being able to identify centers/professionals who accumulate complaints and on whom we can influence, or inadequate work procedures that generate dissatisfaction in the user. But in general terms, most of the claims are related to the process of non-acceptance of the process as work or medical discharges that the worker considers premature, and this is common to all our centers.

F.M.: One of the functions of the department is to organize health care for protected workers who are outside our borders. What percentage does it represent and what is their profile?

M.B.T.: With some frequency, companies send their workers abroad to carry out some work activity and, of course, accidents can arise in that environment. On the one hand, we must guarantee that companies know what requirements they have to meet, for example in the European Union, workers must travel with the European Health Card, but companies must also have communicated the movement. With the requirements met, when the worker suffers a work accident abroad, he or she must go to the device provided in each country, with the Mutual Fund being in any case responsible for paying for this health care. Only when the worker requires health repatriation is it up to the mutual insurance company to carry it out under appropriate conditions; For this we have a concert (Line 900) that the worker can call at any time, and in coordination with this department, the necessary device for repatriation is managed, with accompaniment (medical/ATS) if necessary.

F.M.: To what extent is the socio-labor situation of the country reflected in the benefits that are managed by the health management department.

M.B.T.: It affects us as it could not be otherwise, both in the level of activity we have, and in the effort we have to make with our suppliers to maintain rates at acceptable levels. We have to make permanent efforts to achieve reasonable spending levels in a situation like the one we find ourselves in.
