Document through which the self-employed person formalizes their relationship with a mutual collaborator with Social Security for the management of the coverage and benefits that legally correspond, among them those derived from professional contingencies, the financial benefit for temporary disability due to common contingencies and, where appropriate, the cessation of activity and other benefits attributed by regulation.
