Personal identification documentation
- National Identity Document (D.N.I./N.I.F.) or if you are a foreigner, residence card.
Documents associated with the discharge (only for Common Contingencies)
- Medical leave report.
- Last Confirmation Part issued by the Public Health Service.
- Discharge medical report (if it has already been issued).
Documentation related to the quote
- Proof of contribution bulletins for the last 3 months prior to the medical leave.
Documents to be provided by self-employed or self-employed workers
- I.R.P.F. Withholdings on work income: form through which the applicant for direct payment communicates to the mutual insurance company the necessary data so that it can proceed to carry out the corresponding personal income tax withholding on the direct payments made.
- Activity status statement: social security document through which self-employed or self-employed workers declare the status of their activity.
- Request for direct payment for temporary disability, self-employed or self-employed workers: form through which a worker associated with Fraternidad-Muprespa notifies their request for direct payment through the mutual insurance company.
