How should direct payment be processed?

Personal identification documentation

  • National Identity Document (D.N.I./N.I.F.) or if you are a foreigner, residence card.

Documents associated with the leave (only for Common Contingencies)

  • Medical part of the leave.
  • Last Confirmation Part issued by the Public Health Service.
  • Medical discharge report ( if it has already been issued ).

Documentation related to the contribution

  • Proof of the contribution bulletins for the last 3 months prior to the medical leave.

Documents to be provided by self-employed or self-employed workers

  • I.R.P.F. Withholdings on work income: form through which the direct payment applicant communicates to the mutual insurance company the necessary data so that it can proceed with the corresponding personal income tax withholding on the direct payments made.
  • Declaration of activity status: social security document through which self-employed or self-employed workers make the declaration of the status of their activity.
  • Request for direct payment for temporary disability, self-employed or self-employed : form through which a worker associated with Fraternidad-Muprespa notifies their request for direct payment through the mutual insurance company.
